prepare your dental forms
To ensure that we provide you with the best care possible, please ensure that your patient forms are filled out beforehand. If you prefer to fill out your forms at the office, please come at least 15 minutes before your scheduled appointment.

our brand new building
We are located at 94-779 Farrington Hwy Suite 301
Waipahu, HI 96797. It is across from Times in Waipahu and on the same side as Servco Auto.
Parking is on the first floor of the building, and the main office is situated on the third floor. Our elevator will take you straight to the main office entrance.
